PRIVACY POLICY

Prior to your first appointment, you will be required to complete a confidential health history form. This form must be updated once a year or any time there are significant changes to your health history (surgery, major illness, etc.). You are responsible for notifying me of these changes before the start of your next appointment. The information in your file can only be released with your consent or as required by law. Credit cards on file are encrypted and stored securely via Square.

CANCELATIONS/RESCHEDULING

Life happens. However, as a sole practitioner, 100% of my income comes from you, my valued clients! When you cancel with little to no notice, this affects my livelihood. You are also taking an appointment time away from someone else who could benefit from my care; therefore, the following policies will be fully enforced to ensure my time is maximized:

A valid credit card is required to reserve your appointment. My system does not accept gift cards for this reservation hold. Your card is not charged at time of booking.

If you are unable to make a scheduled appointment, you must cancel/reschedule at minimum 48 hours in advance. This can be done by logging in to the online scheduling system up until 48 hours prior to your session start time. If less than 48 hours’ notice is necessary, please do so via phone call or text. If your appointment time cannot be filled, your card on file will be charged 100% of the full session rate within 1 hour of the canceled appointment time. No exceptions.

Same-day cancelations and no-shows will be charged 100% of the full session rate, no exceptions. Three or more cancelations with 48 hours or less notice within a 6-month period may result in dismissal as a client. Any no-show without any response to my communications will be an automatic dismissal.

Appointment reminders are automated and sent via email 72 hours prior and again via text 24 hours prior to your scheduled appointment time(s). By scheduling with me, you consent to receiving these reminders.

ARRIVING ON TIME

You must arrive at least 5-10 minutes early for your appointment to allow adequate time for intake & conversation. Your appointment time is the start of your hands-on time.  If you arrive on exactly on-time, or late, I can give you only the remainder of your scheduled time.

REFUSAL OF SERVICE & ZERO TOLERANCE

For my safety, I have the right to refuse/discontinue treatment at any time. Reasons may include, but are not limited to: illness, infectious/contagious/unidentified skin conditions, etc... Should your service be refused or discontinued, it is considered a same-day cancelation and you remain responsible for the full session rate. Rescheduling of the session will be determined at the time of refusal/discontinuation.

Suggestive and/or inappropriate comments or behavior will not be tolerated and will result in an immediate end to the session and automatic dismissal. The full session rate will be due.

REFUND POLICY

All retail products are non-refundable.

Prepaid services, gift cards, packages, and the like are non-refundable. If you are unable or unwilling to use any of the aforementioned items that have already been purchased, it is recommended that they be gifted to another who may find them of value. If you wish to “return” an unwanted purchase vs. gifting, no monies will be refunded and the “returned” item(s) will be donated or credited to a charity of my choosing.

GIFT CARD POLICY

The purchase value of prepaid sessions, packages, and/or gift cards will always remain valid. In the event of a rate increase [where the previously purchased session, package, or gift card has not yet been redeemed], the difference beyond the value will be owed at time of service (unless otherwise stated).

By scheduling your sessions with me, you understand and agree to the above policies and information.
